Greater Manhattan Community Foundation Grant Final Report

The insights shared through final reporting help the Greater Manhattan Community Foundation, its Board of Directors, and its Supporting Organizations better understand the impact of collective grantmaking and learn from the work made possible through donor-supported grants. This updated report reflects GMCF's commitment to measuring outcomes, capturing lessons learned, and understanding how these investments are creating meaningful change across the region.

Thank you for taking the time to complete this form and for the important work you do!

Section 3: Who You Served

Populations Served

Please indicate the primary populations served through this grant (check all that apply):

Section 4: What Changed — Impact Indicators

This section asks you to name up to three goals you had going into your grant for the population you serve. Discuss the degree to which that goal was met and share whatever evidence you have to support this belief.

For this purpose, you don’t need a formal evaluation. Observations count. Conversations count. Even honest uncertainty counts. None of these require a survey to report on, though over time, building toward that kind of evidence strengthens your program and your case for funding.

Section 5: Learning & Reflection

The strongest programs are built on honest reflection, not just success stories. We want to understand what you learned, even when things didn’t go as planned.

Section 6: Story of Change

Numbers tell us what happened. Stories tell us what it meant. Please share one specific, concrete example (a person, a family, a moment, etc.) that shows what this grant made possible. You may change names or details to protect privacy.

Section 7: Sustainability & Next Steps

Section 8: Public Acknowledgment
